Reporting a sexist incident form

The purpose of this form is to record any incidents of sexism in Football. It will be used to collate the incidents in a consistent manner and to report to the appropriate bodies.  Women in Football will treat all the information you provide sensitively. Please note you also have the option to remain anonymous. If you are unsure about any elements of this form or if you require assistance completing it, please contact Women in Football on info@womeninfootball.co.uk.

If during a match please estimate as close as possible the minute in the match that it took place.

Please include league or competition and venue of match, if applicable. If a workplace incident please indicate the environment it took place in.

Try to provide as much information as possible. What was said, names of those involved if known. Context of the incident, for example in an open plan office, chanting at a match, sexist signage.

Please provide the stand/block/row/seat number if at a match. Please indicate n/a if a work place incident
